Whenever an emergency occurs involving a minor, health care providers need the permission of a parent to treat the child. If a parent can not be reached, written consent is required before the child can receive emergency medical treatment. Anyone in the community is welcome to stop by the main entrance of the hospital and pick up a blank form. The forms should then be filled out by the parent(s) and left with each child’s caregiver in the case a child would need emergency medical treatment when their parents are not available.
The Emergency Treatment Authorization Form for Minors is a useful tool to have ready in the case a child needs emergency medical treatment when away from their parents. These forms should be filled out and left with the care giver for each child. The caregiver can then present this form to the medical provider so there is no delay in the child’s medical treatment.
