Emergency Treatment of Minors

Whenever an emergency occurs involving a minor, health care providers need the permission of a parent to treat the child.  If a parent can not be reached, written consent is required before the child can receive emergency medical treatment.  However, if a physician feels immediate treatment is necessary to prevent death or serious injury, treatment will be given. Emergency Medical Treatment Forms for Minors are available at the main entrance desk at Veterans Memorial Hospital.  Anyone in the community is welcome to stop by the main entrance of the hospital and pick up a blank form.  The forms should then be filled out by the parent(s) and left with each child’s caregiver in the case a child would need emergency medical treatment when their parents are not available.

The Emergency Treatment Authorization Form for Minors is a useful tool to have ready in the case a child needs emergency medical treatment when away from their parents.  These forms should be filled out and left with the care giver for each child.  The caregiver can then present this form to the medical provider so there is no delay in the child’s medical treatment.

Please feel free to stop by the main entrance of Veterans Memorial Hospital anytime to pick up these complimentary forms or Click Here to download the Emergency Medical Treatment Form for Minors.
