Our registered nurses and certified home care aides travel the area to offer assistance to help you remain in your home for as long as possible or when you just need some extra help while you are recovering from an illness or recent surgery, often times after being hospitalized.
Home care services cost only a fraction of hospitalization and nursing home placement. We are dedicated to your well-being and want to ensure that you safely remain in the comfort of your own home.

Nursing Services
Our registered nurses will assess your condition and develop a plan of care tailored to meet your needs, which may include:
- medication set-up, administration, and education
- dressing and wound care
- injections and IV therapy
- catheter and ostomy management and education
- physical assessments
- overseeing in-home rehabilitation program
- patient and family care instruction
- communication with your medical provider
- improve home safety
- coordinating medical appointments as needed
Home Care Aide Services
Certified Home Care Aides can assist you with:
- Personal care, including bathing and dressing
- Hair care
- Home exercise programs, especially to prevent falls
Therapy Services
Occupational Therapists provide in-home evaluation and treatment to help you develop and maintain the ability to perform the essential activities of daily living such as:
- Bathing
- Dressing
- Preparing meals, etc.
Physical therapists evaluate and provide rehabilitation to home-bound patients following an illness, surgery, or injury, which may include:
- Exercise programs
- Posture balancing training
- Daily living assessments
- Recommendations for home safety modifications
Speech therapists provide in-home evaluation and treatment for speech and swallowing problems, including:
- Assistance in producing speech sounds
- Language comprehension and expression
- Swallowing techniques
- Diet recommendations
Insurance Coverage
Our nurses will work directly with you to coordinate benefits and provide appropriate, cost-effective care to meet your needs. Listed below are examples of the payer sources our agency utilizes.
- Medicare and Medicare Advantage Plans
- Medicaid Managed Care Organizations
- Private and long-term care insurance
- Sliding fee scale
Private pay is also an option.
Referrals can be made by family, physicians, neighbors, friends, and other healthcare facilities. Oftentimes, our services go unnoticed by larger hospitals and may not be referred to patients who need these local services.
Whenever being discharged home from a health care facility, YOU can request that VMH Community and Home Care provide your home health services!
RN staff are on call 24 hours a day, seven days a week, to help with urgent needs that may arise. Community and Home Care is Medicare-certified and a member of the Iowa Association for Home Care. Veterans Memorial Hospital began managing the Allamakee Public Health Department in 1995. Our nurses, home care aides, and receptionists Community and Home Care offer these in-home services listed above, plus many public health services to the communities each month.
